March 12, 2014

Mary Lou Osevala of the heart failure program of Penn State Hershey Heart and Vascular Institute, recently received the Heart Failure Award from the Preventive Cardiovascular Nurses Association (PCNA). Osevala’s award submission described the implementation of the nurse practitioner transitional care model that is used in the heart failure program, to help reduce hospital readmissions.

The PCNA awards committee commented that the award submission had “the most comprehensive approach, from a theoretical framework, and positive program outcomes,” compared to other award submissions that were received. As a result of winning the award, Osevala has been invited to prepare an abstract for a poster presentation at the PCNA Annual Symposium.
