Insurance deadline Jan. 23 for graduate assistants, fellows

University Park, Pa. -- New graduate assistants and graduate fellows for spring 2006 are reminded that they need to notify the Student Insurance Office about their insurance preferences for the spring semester. These students must complete and submit all health insurance forms online this year by Jan. 23 at http://www.sa.psu.edu/uhs/enrollinsurance/ online.

Penn State subsidizes the premiums for graduate assistants and graduate fellows for their dental and vision plans, as well as for the Penn State Graduate Assistant and Graduate Fellow Health Insurance Plan (a medical plan). The University deducts the student's contribution to the premium expense from their monthly paycheck (September through May). Enrollment in all three plans is automatic for single student coverage.

If graduate assistants and graduate fellows do not want to take advantage of the medical, dental and vision insurance plans being offered, they must complete an online declination form. If they want to enroll spouses or dependents in the three plans, they must also go online and complete the dependent enrollment form. The declination and enrollment forms can both be found at http://www.sa.psu.edu/uhs/enrollinsurance/ online.

Health insurance continues to be a mandatory requirement for all international students, including graduate assistants and graduate fellows and their spouses and dependents. If international students have other health insurance that meets the University's requirements, they must complete a waiver application at the Student Insurance Office in 208 Boucke.

All waiver forms, dependent enrollment and declination forms must be completed every fall semester. If students have questions, please call 865-7467. Or visit the Student Insurance Office Website at http://www.sa.psu.edu/uhs/basics/insurance.cfm online.
